Waiver

INSTRUCTIONS FOR PARENTS: 
PARENT NAME -and- PARENT BIRTHDAY must be entered first.
Minor Children's information will be entered after parent's information. 

IMPORTANT!
1) Waivers for minors (those under 18 years of age) must be signed by a PARENT or COURT APPOINTED legal guardian.
2) Waivers signed by a friend, relative or group leader WILL NOT BE ACCEPTED.
